I just think that things should work the way I expect them to.

On full manual yes but if you have auto exposure the camera will expose to let in the necessary light. If you set manually 1/40" and F4.0 the image would have been underexposed. Since the setting was for F4.0 and auto shutter the camera would have bumped up the ISO until it reached the upper limit then lowered the shutter speed ignoring the minimum 1/40" internal setting and used 1/15" to get the correct light.

BTW it is useful to remember that you have the auto ISO set up to 3200 when you want to deliberately do long exposures with a tripod at night for light trails etc. You have to remember to reset the auto ISO to the lower sensitivity (200?) to force a longer exposure.
