5D3 Warranty Card - What does it look like?

Ok, I've got a strange question.  I took a chance on one of the recent deals to upgrade my 5DC by ordering a 5D3.  It was listed as new and USA model and there is a warranty card in the box, but it is different that what I was expecting and just want to make sure it is legit.

Could somebody describe, or take a photo, or scan their card so I can make sure everything is good?  (Obviously I don't need to see any personal information)

I figured worst case, I could fight it out with the company based on what they were selling via my credit card.  Hopefully there isn't a problem.
