So here's the current status:

It looks like over tightening was likely the issue, though the damage did not occur entirely on the night the thing actually broke. I can't rule out the possibility damage started to occur before I got the camera, but there is no doubt I at least finished it off. There is also a possibility the thread was too long for the bushing, causing large torque on a small part.

I sent the camera to Olympus for repair. This is probably the expensive option, but I have a trip coming up at the end/beginning of the year and need the camera functional by then, and don't know who else can do it.

Apparently I should get a QR system to avoid this in the future. I was trying to avoid this because of the weight and price. For example (and what I am currently looking at):

Really Right Stuff Olympus Body Plate    $60  48g

Really Right Stuff B2-FAB-F 38mm clamp $60  55g

Really Right Stuff E-M5 grip (optional)     $65 46g

Total is $185, 150g. This would make my camera heavier than than my two person tent or backpack for outdoors purposes, and on par with my sleeping bag or down coat. Assuming the money is no issue because it's cheaper than having the camera repaired again. Obviously there are lighter and especially cheaper options, but I'd say that if it needs to be done, there's no point cutting corners (however I may still wuss out and go smaller and cheaper).
