OP said he was turning tripod and not the fastener screw? Yes, I do that sometimes, turning the head a little bit if the camera gets loose on the mount.

Doing it in the dark, with gloves in the cold ....well I could see that if you were holding the arm on the tripod that you would be putting a lot of leverage onto the screw. Honest, but easy mistake.
