D4-500mmVR and Birds in Flight

When I use a tripod, which isn't that often, I will use a Wimberley Gimbal type head.

All of these shots were handheld.  I have a Black Rapid strap that is attached to the tripod mount on the lens and just lift it up when the birds are coming.  It can get a bit tiring after a while but I have found that it works well for me.
