Ah... so it's possible I did cause this... well, maybe I'll have to get me a QR after alll. Dang it.

If the bolt is too long (for example if it's missing its rubber/cork padding) then once it hits the bottom of the tripod socket if you keep turning it simply pops it off. Looks like the base of the camera around the tripod socket is bent upwards which seems to support this.

It also might be possible to do this by over-tightening, especially if you were screwing it directly onto the tripod rather than using a quick release plate as you get a fair bit more mechanical leverage doing that (the circular scuff marks on the bottom of the camera make me think you may have done this?). Again doing this would bend the metal around the tripod socket as it's forced out.
