I'm wondering if you had given your flash sufficient time to "charge up", either not enough time upon start-up, or too soon after shooting a previous picture.  Was this the only picture you took?  If not, were others also as badly underexposed?  Did you take these from the same vantage point? I also like earlier suggestion that specular reflection of the flash from the window has caused underexposure.
