I use LR4, Imagenomic's Portraiture, and Photoshop. I check the NEFs for WB, exposure, noise, blemishes, etc. in Lightroom. Then I export through the Potraiture plugin to the hard drive as either TIFFs or JPGs. Then I look at them in Photoshop to tweak skin tones and USM. Portraiture does no facial distortions, it simply smooths the skin and intensifies or diminishes color, saturation, etc. Default settings work fine, although I've de-tweaked them a bit down. There are a few portraits in my gallery you can check.
