If it's the same as the original Note that I have, then you just do the following:

1. Plug a USB OTG cable into the micro usb socket on the Note.

2. Plug the card reader and card into the standard USB socket on the other end of the  USB OTG cable.

It's as easy as that and the card contents will be accessible to all apps and file managers. The USB OTG cable can be bought for a few quid on ebay, there is no need to use the expensive Samsung branded cable. You can also plug in mice, keyboards etc this way. Devices that need lot's of power may not work.
