You probably just think you have taken it off. Throw the filter away and you can never be confused again... And your mirrored through the optical axis ghost lights will be a thing of the past (probably).
If you get it with images that were really taken with no filter, the cause is the shape of the front most element in the 70-200mm f4 L IS USM, which has quite a flat back surface. But I am not convinced yet that that image you showed was not with filter.
I'll take that on board. The shot is a work in progress. Looks like good weather today, so I'll go back there tonight and give it all a thorough run through.
