avoiding a fogged lens

I've got a January wedding booked, and I want to be proactive about the possibility of a lens getting fogged. I don't need the discussion to be about my backup equipment. I'm all good there. I just want to know about how to avoid it.

Would it happen when coming inside from the cold? Or vice versa? Should I have one set of lenses just for inside, and other lenses for outside?

If it happens, will the fog go away on it's own and possibly be useable again later?

Can I learn from anyone's experience here?
