In your example an eye is much to small for the camera to follow.The chances are most of the head will still be under the original focus point so the camera will not follow and refocus on the eye

Fair enough; in my OP I was referring to shooting with initial focus on the person's head (and it being big enough in the viewfinder, slightly larger than the focus point rectangle). What, if any, information from the adjacent points is being used and how? That's what I'm trying to understand.
