Lighting set-up for couples

This is my first time setting up a couples portrait. I have 4 speedlItes, 2 stands, and 2 umbrellas. I have read about people using 1 light just off axis. Would you elevate the light? What are the pitfalls of trying to do a basic loop lighting set-up with key and fill?
