Camera Settings for Multiple-Exposure Combos ?

When you record multiple exposures of differing brightness levels for the purpose of combining them in some way in software in order to get better results (HDR, Fusion, etc.), I was wondering:

(1) How many individual exposures do you prefer to record ?

(2) What is the spacing (in camera-setting Exposure Value) between those individual exposures ?

(3) How do you like to set the (camera-setting) Exposure Values ?

(4) Do you over-expose all of the shots, or just some of them ? If just some of them, which ones ?

Note : By "camera-setting Exposure Value", I mean the overall image-brightness as it is affected by ISO Gain setting, as well as affected by incoming light levels, F-Number, and Shutter Speed setting
