To check, shoot two images in manual mode, one at f/2.8 and one at f/4.0, with the same shutter speed for both. Looks like you have a bad copy. As for the aperture issue, what tool are you using to measure brightness/EV between the f/2.8 photo and the f/4.0 photo? I have LR4.
