eposure issue with e-m5

I'm having an issue with exposure both with manual focus lenses and 45mm f1.8:

using esp metering and aperture preferred a lot of images a under exposed (I'm shooting raw+fine/jpg)

I'm shooting single shot af. Here are three sample jpeg (the fifth one looks like the first):

I'm really quite confused. Also my iso 200 images seem to have a bit of grain when viewed with rawtherapee (but maybe that is normal with 0 noise reduction?)

Anyways i was kind of hoping someone who is not as clueless as myself could
look at the exif data and possibly provide an explanation.

Exposure compensation is set to 0 'cept when i try to correct for the under exposure; i can make the raws available if interested. metering is EPS (which I
cannot believe is having this much issue but maybe it can't pickout the white
point correctly ?)

Maybe there is an issue with auto gradient ?
