Best way to carry large lenses?

Blackrapid strap with a quickrelease plate on the lens handle, carry the monopod in your hand with a quickrelease adapter on it. That's how I shoot with my 120-300/F2.8. Putting the two together is awkward, and I tend to bump things with the 'pod. The mirror makes enough noise; I don't need to be smacking the monopod on chairs in the back of the theater, too.

The monostrap connected to the handle of a long lens tends to balance very well and it's easy to cradle the lens in the small of your back with one hand on the body, keeping it mostly out of harms way but ready at a moments notice. It takes a couple of seconds to unhook the carabiner from the QR plate, but it's no longer than having to take a big kit out of a bag.
