I did some experiments with defog and suspect that its main method is to expand the histogram which means to deepen shadows and raise highlights if these are not already near the limits. Obviously this would do the most effect if the image was one which contained low contrast. If the exposure is already at either end of the histogram then it expands the side that is not at the limit.
