By skill and experience. DOF scales, smaller apertures, fast shutter speeds, higher ISO's etc all can help.

If a photographer can not manual focus on a moving object, than I'd say they need to hone their craft a lot more. Obviously AF is a godsend for pro's out shooting sport and editorials, but still, competent manual focussing = photography 101.
