7D, 6400. My alma mater's gym is poorly lit, and to get 1/400 at f/2.8 (which I've found to be the slowest I can go to get sharp basketball shots) I usually need ISO 6400 (I'm using a 17-55 f/2.8). With a 50 f/1.8 I can use 3200, but that lens doesn't focus quickly enough.

In Salem, VA for the NCAA Division III Final Four, the place was much better lit. I had to use a Sigma 70-200 because I couldn't get a floor pass, and I found 1/640 worked better. But it was 2 stops better lighting, and auto ISO was giving me 2000-3200.

With flash? I depends. I've gone to 3200 when I need fast recycling time, but I prefer 800 or better.
