I too have noticed a back focus under 'off white' artificial lighting. Like you I wasted hours on focus charts (useless 45 degree angled charts, LCD moire images etc!). For me thing that did it was the LCD moire charts (google it). Using an LCD screen with white lighting the AF was spot on with no tuning (referencing against live view shots).

But 'yellow' lighting causes back focus. So if I know I'm indoors under artificial light using primes SOMETIMES I'll switch on a default AF tune or use live view if possible.

But now I just try to ignore this tuning obsession and take photos!
