According to PPG it is his only SDM lens. It may not be - but I can only surmise based on the information at hand.

Ridiculous. Any camera needed to be turned off for lens switching should be trashed.

Not ridiculous. Every manufacturer recommends you do this.

Because they are faulty. It is certainly a potential issue - but probably the least likely as the lens was working ok when it was sent to him.

Shouldn't the camera die first?

Not necessarily. The lens circuit may be less tolerant of voltage issues than the camera.
