do I have a flash/camera problem?

Last weekend I did a workshop about making pictures inside a room. We practiced a lot. At one time I had a problem. The equipment is quite new, so new to me.

We were regularly changing settings of the camera as well as the flash. After playing with the settings pictures were suddenly extremely overexposed. I went through the settings again and changed them, no change. At home I saw in the EXIF-information the flash was not fired, though it was.

By switching both camera as well as flash off, waiting a minute, and on again the problem was solved.

A communication problem between flash and camera?
Any idea what might have happened?
Should I start worrying?
