Repair Center Address/Procedure? x100 exposure issue

Hey folks. My x100 has succumbed to the exposure issue after months of use. I was looking up how to send it in today and I see that they say that I have to send in a warranty card. hmmmm. I have a receipt from BH Photo, but not sure I have a warranty card anywhere. What have others done? I was going to send it to the NJ address.
