The shim goes between the chromed mount (that the lens attaches to) and the body of the adapter. The mount ring is usually held by three or four obvious screws; remove the screws, fit a shim, replace mount, test. Repeat until happy with infinity focus.

Aluminium (kitchen) foil is good for thin shims but I often use paper if thicker shims are needed. Hard paper (tracing paper, say) is best.
