580 EX problem

I have a 580 EX unit and I tried to use that little plastic rectangular piece that pulls out and goes down over the top of the flash unit. But when I used it on TTL mode I couldn't seem to get any light out of it. Even with +3 I couldn't light my subject. Then when I put it back inside it worked just fine?
