Custom Profile Problem

I recently ordered 2 custom printer profiles from a company for the same printer/ink but just different papers. One worked perfectly, but the other just looks completely wrong. Interestingly it looks just as bad after only applying the profile in Photoshop and before printing, so it doesn't have to do with my settings. The company tried remaking it but it was the exact same. I attached a cropped sample of a photo showing what it should look like and what it looks like after applying the profile. It prints the same way it looks here in the after photo. Anyone have any idea what is going wrong? I printed the original targets using Adobe Color Print Utility but just printed a second set using Photoshop CS4 with color management turned off. Any other ideas?
