60D sometimes doesn't "recognize" 420EX flash

I have had 2 instances where there is seemingly a communication problem between my new 60D and my 420EX flash. The flash fires, always at maximum output, totally blowing out the image. If you review the image at look at the info, the 60D indicates that the flash was not fired, though obviously it was.

I'm not sure the exact sequence that creates this, but as I mentioned, I have seen it twice in my 2 weeks owning the 60D. It may have something to do with leaving the camera and/or flash turned on but letting them go to "sleep". But I have not been able to duplicate it when I try. Re-connecting the flash and/or powering down the camera and flash seems to fix it.

Anyone heard of this problem before? I witnessed this using the EFS 17-85 lens, so this isn't the over-explosure issue fixed in the recent v1.0.8 firmware update.
