I am using several Mamiya 645 lenses on my Nikon D300 and D700. Based on picture you posted I would say:

1. You are very close to me. I am on corner of Ave of the Stars and Pico.

2. Your problem is in adapter. I am using FotoDiox Pro and am very happy with result (below is one of them, taken with Mamiya 200/2.8 APO). Adapters need to have perfect dimensions to line up focal planes.

3. You might be interested in some of my Mamiya lenses.
