New cameras are still being released without an orientation sensor. Other systems use the sensor of the lens-OIS if that is available but have no in-body backup for when non-OIS lenses are used. Without an orientation sensor you lose minutes manually selecting pictures after every shoot.

How hard would it be to include such a sensor inside an SD-card or a micro-SD adapter and have the card modify the EXIF orientation-field of each newly written .jpg?

As some SD cards are inserted at the bottom of the camera and others at the (right) side, a system to let the card know which orientation is horizontal should be in place. You could add an extra hardware switch (cf "lock" switch) or put a simple configuration file on the card.

I would happily pay $10 extra for such a card.
