Do you know what this fault with a 70-300 IS lens is?

I have had the Canon 70-300mm IS for a while now but recently I did give it to Canon for modification.

Since then I have not used it much but on two separate occasions on first fitting the lens I have had the following problem which is visible through the view finder but if I remove the lens and check the optics there is no sign of fogging or such that I can see. If I keep using it, it does improve but quality is still not great.

Is it fogging, the IS system or focus? On focus I tried to manually focus with no better results.
