When NOT to use IS on 70-200mm f2.8L IS

Totally understand using IS when hand-holding this lens and turning it off when using a tripod, but what about when using a monopod? Do you use IS or not? and do you change to IS mode 2 when switching to portrait mode?

Tried this in the real world but my results are too variable to make any hard and fast rules about it.

How do YOU use your IS on your 70-200mm f2.8L IS? Appreciate any "real-world" experiences.
