Nope, but it was clearly taken west bound so I would surmise either 36,000ft or 38,000ft.

Also, there's a "hole" in the clouds on the upper left side of this

That is merely a break in the clouds my friend! What you see here is layer upon layer of clouds from a slanted view into them and where there's a break in the clouds you see more air beyond (you cannot see the land below in that break because its so many miles away).

Anything else you may want to add would be of great interest . . .

The Earth spins at approx. 1000mph and when flying west bound you're flying into the setting sun at approx 600mph. The end result is that the sun sets rather slowly.

Factoid: Staring into a setting sun for six hours on an afternoon transcon sucks.
