Use of Lens Hood

Lens hoods minimize flare and are also protective. But I've seen many pros do away with the hood, at least some of the time. Look at Art Wolfe's current TV series, for instance. Forgetting the protection for a moment, when is the hood necessary and when is it not? For example, could one say that the hood doesn't help if the sun (light source) is in the 180 degree circle behind the sensor plane (assuming no major reflecting surface)? If so, what if the sun is high but behind? Would you say it's necessary when the sun or light source is in the 180 degree circle in front of the sensor plane? What about indoors: does the light source have to be quite strong to cause flare inside (for example, what of typical house-lights?)?
