40d modes/shutter speed

Can someone explain what is influencing my shutter speed on the 40d...

(Tamron 28-70 f/2.8)

When shooting full auto (green box), The camera is using ISO400, F/4, 70mm with Flash and gives a shutter speed of 1/60.

When shooting Av, I set to ISO400, F/4, 70mm and manually extend the flash, and I get a shutter speed of 1/10.

Why the huge difference in speed when all the settings are the same? Am I missing something?

I have speed issues in general when shooting indoors with natural light from windows. Even at f/2.8, ISO200 or 400 in Av I get shutter speeds too slow to handhold.

What am I doing wrong?
