Help With Retouching Photo

Hi. I took the photo below for a friend. The baby's one year old birthday is coming up soon. So I plan to make the photo look more interesting by doing some retouching on it (maybe a different background?). Then print it out, frame it, and give it to my friend as a birthday gift. The problem is that I'm not very good with photoshop. So can you help by showing some of your creative skills?
