Failures of Design

Challenge #6 in the Jaysen dog-foods challenges series. Hosted by Jaysen Marais.

Sometimes, despite the best of intentions, man-made creations either don't do what they're supposed to or conflict with other objects and/or their environments in such ways that we can't help but shake our heads. I'm specifically looking for examples of counter-productive human creations here, no poking fun at the natural world itself. Bear in mind that some designs are elegant yet unintuitive, so be careful when pointing the finger.

swimming along this barricade, the duck worked iteself into a mini-frenzy

This barricade was erected to prevent leaf litter moving from one section of the ponds in Regent's Park (london) to another. Extending only about a foot above the waterline the barricade (which stretched across the width of this stream section) prevented the water-birds from paddling past it. However the birds could still see through it and would paddle back-and-forth, growing increasingly agitated that they couldn't get to the other side.
